What is Form SL-2?
Form SL-2 is known as the Diligent Search Report. All new businesses require this document. This report may be claimed according to the California Insurance Code Section 1763. You or a licensed broker or agent must sign this document.
What is Form SL-2 for?
The form provides data about the placed coverage and describes your efforts to establish the risk with the licensed insurer.
When is Form SL-2 Due?
Form SL-2 covers the search period of 60 days.
Is Form SL-2 Accompanied by Other Documents?
This form cannot be filed separately. It must be attached to CDI Form SL-1 that is the Report of Placement.
What Information do I Include in Form SL-2?
Completing this form you must provide the following information:
- Full name of the individual;
- Name of organization;
- License number;
- Name of insured;
- Address of insured;
- Description of risk;
- Location of risk;
- Insurance coverage type;
- Full name of admitted company;
- First & last name of company representative and telephone number;
Where do I Send Form SL-2 after Completion?
When the form is completed, file it with the Surplus Line Association of California.
